Documentation Services

Many of our clients choose X-Plain products that document patient education. The reasons are varied and include meeting regulatory standards (e.g. JCAHO), managing risk, enhancing staff communication, or tracking usage and comprehension.

X-Plain products that offer documentation services do include the price of documentation in the subscription fee.

The data saved by X-Plain software depends on the product. From simple to more complex, X-Plain documents:
  • Usage (educational session started)
  • Completion (educational module completed)
  • Comprehension level (questions answered correctly)
  • Users’ responses to questionnaires
  • Users’ feedback, comments, and rating.
  • Data for evaluation or experimental research, including data from repeated measure designs.

Often, documentation is the weakest area of the patient education process. Unfortunately, organizations that do not document patient education activities may give the impression that such activities did not happen. ..Documentation is done for several reasons including the following:..
  • Serves as evidence of teaching to fulfill regulatory requirements
  • Minimizes professional liability
  • Tracks time devoted to patient education
